3-on-3 Basketball Event Sign-up Form

Sign up below for our 3-on-3 Basketball event. If you are a member of a team, please supply the team name where indicated.

If you are part of a team, please enter the Team name here. Teams are comprise of 3 members. If you are a team you need to file this form out for EACH team member and make sure to add the Team Name for each player. If you are a single player you will be assigned to a team and you do not need to fill in the Team Name.
